Accident Report Certification Form
Public Chapter 111, Acts of 2019
Public Chapter 111, Acts of 2019, became effective July 1, 2019. The public chapter provides that personally identifying information contained in a motor vehicle accident report is confidential and not open to public inspection under the Tennessee Public Records Act, except to a person named in the accident report, such person's agent, legal representative, or attorney, once that individual certifies that he/she has permission from one of the named parties to the accident, to obtain the personally identifying information in the report. "Personally identifying information" is defined as street addresses and zip codes, telephone numbers, driver license numbers, and insurance information.
For Insurance Agencies ONLY. The insured's agent must fill out the Accident Report Certification form, attach a copy of the agent's photo ID and fax to the LaFollette Police Department at 423-562-4281. If you have any questions, please contact the Records Department at 423-562-8331.
